i can send you mine if you want - we learned how in a professional practices class, and it is geared towards art jobs.

it is a bit of a tricky situation. i am applying for a job at adult day care to work as an assistant activities therapist. so i need to show that i have art skils/art therapy/etc. but i googled and could not even find a normal template without paying. i know there is one in word but alas i do not have word.

I have mine, which is set up fairly basically and should be sufficient for that you are looking for.
