If you have cooked it at least 15 minutes at home at at least medium heat and you keep it covered, you should be fine to transport it and then continue cooking. By covering it, you insure that no pathogens will be introduced to the dish.

Can't you cook it fully at home (even the night before) and just reheat it somehow when you get to work?

I concur; I would cook it completely, then reheat. You can zap it in the microwave when you get to work, then put it in the crock on "keep warm," it should be fine until service.

Also, the recipe you post only takes 2 hours. I you have a way to store the milk that is added later, you could bring it uncooked in your crock pot and let it cook while you work.

Either make and home the day before and reheat, or:
Cook your macaroni the night before, rinse with cold water so it doesn't stick, drain, and pack up for transport. Transport the ingredients and crock pot to your work, with the milk, cheese, and macaroni packed in an insulated bag or cooler. Plug in your slow cooker at work, and do the rest of the assembly/cooking there.
