My mother's 18 yr old cat Destiny has disappeared. We found him the summer before I was in 8th grade under the hood of Mom's car.He is an indoors-only cat and it would be pretty much impossible for him to sneak outside. We just lost Judas to thyroid problems. We had him put to sleep because in his last days he was finding dark private places to die. Then he lost the ability to stand. We didn't want him to suffer. Your cat might still be alive, but hiding himself from the other cats. Its a primal urge. I hope you find him.

gah, how heartbreaking! I hope you find him, and that either he can get whatever health care he needs, or that he was able die in peace and without pain. I know it never seems like long enough, but at least 18 years is a nice long time for a kitty. I'm still hoping he turns up alive, though! *hug*
