There has to be a way to measure the viewership of a TV show other than Nielsen ratings, which I think are tremendously broken and inaccurate, especially for a show with a potentially narrower appeal than the typical Nielsen household

Arbitron has a device called a "Personal People Meter" that uses encoded signals in TV, Radio, and to determine exactly what people are watching (or being exposed to). Of course, it's not perfect, it still requires a panel of participants, they don't work for free, and each radio/tv/internet entity needs to incorporate a transmitter into their signal.

And, really... Dollhouse is a show for a niche market. That doesn't make it a bad show. However, it doesn't have mass appeal. If Fox is looking for mass appeal, I'm not sure what exactly they could have done with it.

They do issue multiple sets of ratings after a week or two, and there are different numbers for people who watched within 24 hours or within the entire week. I'm not sure the networks really pay all that much attention to them, though (possibly because DVR users are more likely to skip ads)
And, really... Dollhouse is a show for a niche market.It is, but I'm pretty sure that market is larger than the 2 million or so that Nielsen has been reporting watched it. I also think that market is more technologically savvy and more likely than the general population to have some sort of DVR device (it's 25% of all Americans, and I would guess much higher among fans and potential fans of Dollhouse). Though that tech savvy audience is also more likely to choose alternative methods of viewing - Hulu, iTunes, Amazon, and unfortunately, piracy ( ... )
