John is disappointed when fat teenage Dean presents as an omega. His heat cycles will be a nuisance on the road, one that John just doesn't want to deal with. But he's heard that high body weight can affect an omega's heat, lessening or even suppressing it depending on the level of obesity. With the way Dean is ballooning, it would only take a little encouragement to make him massive. Taking care of an obese omega would be much easier than dealing with one in heat. And in fact, Sam will be old enough soon to handle the responsibility of Dean's care.

Accidentally picking up some mojo from a fertility idol led to Dean getting pregnant with high end multiples (at least four but feel free for a higher number) that he managed to carry for a full 40 weeks. Dean being Dean meant that he overindulged the entire pregnancy and put on a lot of weight. Once that the pregnancy has come and gone Dean is left with a body he finds less than desirable somehow managing to be both fat and have an abundance of loose stretched out skin. He's not keen on dieting and knows the more he loses the saggier he'll just become so he makes the decision to try and get back up to his pregnancy weight so he feels more comfortable in his own skin... plus who doesn't love pigging out?

Sam knows he's never going to get his body back. He knows that five kids under four, with another one the way, is hectic. He's not even sure how they found the free time to get him pregnant again. He also knows that even though he should know better he won't be able to tell Dean no if he wants him knocked up again because he gets off on it just as much as his brother does.

The best omega broodmares weigh at least 600 pounds. They should be too fat to do much more than lie on their backs through their breeding, pregnancy, and labor. Dean has birthed many healthy litters and enjoys his life as a broodmare.
Up to you if Dean has a favorite breeding partner, who always brings him plenty to eat while he's being knotted.
