i would go with the *just say thank you* route, and then donate it to salvation army
even though it is not what you would want from her (or anyone) obviously she has very little awareness of those around her, and even less social graces......you are not going to be able to explain to her that this offends you without offending her........why bother? if it was someone more important in your life like, well,......say ME for example, then i would tell me that though you appreciate the thought, the gift makes you uncomfortable and you think i should save it for someone that can more fully appreciate it............but odds are that i would not ever give you a religious based gift, as i am fairly non-religous myself, and she will simply see it as an attack on her religous beliefs

Maybe she'd get the hint? Or, you know. Just tell her exactly what you wrote here. "Thanks - I really do appreciate the thought, but I've got very different beliefs than yours, and it is awkward for me to accept this gift. But, like I said, thanks again for thinking of me." Type thing.
I know how you feel, but try to remember that her religion tells her that she is responsible for the soul of everyone she meets, and will be held accountable for them when she dies. It's not a matter of social graces, really, it's CYA with God.
Go the Salvation army routine. When someone is that passionate about a religion, there is no reasoning or changing their minds. Particularly when they're your boss.
If she's accountable for MY soul, the poor dear might as well give it up now, she'll never make it through the pearlly gates! I'll be dragging her into hell with me.
