filing for name/gender marker change

I've been trying to find the paperwork for my name change online. I seem to only be able to find the instructions, but not the actual paperwork. Where would I look? The city hall website perhaps?

And where and how do I file to change my gender marker on DL and such?

I am in Texas if that matters.
