neither, really. "have you got a" is much more likely to come out of my mouth. but actually, come to think of it, i think it would sound more typical to me to hear "use a condom" in the middle of a sentence, and "wear a condom" as a directive with no speech preceding or following it. "use a condom" would sound weird in that context. and "be sure to wear a condom" sounds a little odd to me too.

And yes, "wear" does conjure up some funny pictures. "Why, that's a lovely latex ear-warmer you've got there..."
